Health Insurance Portability and Accountability Act (HIPAA)

Freedom Services, Inc. is committed to protecting the privacy of our clients and plan participants. We consider maintaining the confidentiality of nonpublic personal information important to our mission of providing quality administration services.

All healthcare providers and health plans are required to follow standard federal privacy regulations. These expanded regulations are part of the Health Insurance Portability and Accountability Act (HIPAA). HIPAA will help protect your health information in these ways:

  • Defines individual health information - an individual's health and billing information in any format - paper, verbal or electronic.
  • Defines healthcare organizations - physicians, hospitals, health plans, claims clearing houses and many other organizations that are involved in the health care delivery process.
  • Defines individual rights over your health information - HIPAA provides you with rights to help understand and control how your health information is being used.

Personal Information We Collect

  • Any information that is provided to us through the completion of the following forms: claim forms, enrollment forms, or any other form necessary to administer coverage or pay a claim.
  • Any information from others that is necessary for us to properly process a claim or otherwise complete a transaction requested by our customer or plan participant.
  • Any information our customers or plan participants authorize us to collect from others.

Personal and Health Information We May Disclose and to Whom

  • We do not disclose any of the above-described personal information to any third parties, except as required by law, and/or if authorized by the individual.
  • For various business reasons, we may need to discuss an individual's personal information to our affiliates or others. These disclosures may be made to others for the purpose of performing administrative services, helping us administer or review a claim or as requested by a governmental agency.
  • We may disclose the above-mentioned personal information to other nonaffiliated third parties in order for them to help us provide superior product and services. Should we determine that these disclosures are necessary, we will seek assurances that these third parties will not further share the information beyond its stated purpose.
  • We will not share any individual's health information unless allowed by applicable law and/or the individual has provided us the appropriate authorization.

Confidentiality and Integrity

  • We use physical, electronic and procedural controls, including physically secured areas and computer controls.
  • We assess the integrity of our systems through auditing, monitoring and data management processes.
  • We have policies to direct and procedures to limit the access of an individuals’ information.
  • Our employees are continually trained on how to keep an individual’s information safe.
